01LIFE CHANGED
The problems kept piling up.
Before a traumatic event, this member lived independently and managed their own life. Afterward, they were left with a physical disability, grief over the life they had lost and a need for more help than they felt comfortable requesting.
They lived alone and had very little contact with people outside the home. They could no longer drive, and their equipment made grocery shopping difficult. Transportation rides sometimes left them waiting for hours. Appointments were missed because they were forgotten or because transportation authorization had expired.
Inside the apartment, everyday tasks had become overwhelming. The member wanted help organizing and cleaning but could not even get the garbage outside. Spilled animal food and accumulated trash began attracting insects and creating additional problems. They also lacked safety equipment and did not know how to get the medical equipment they needed.
02WE STARTED HERE
First, we handled what mattered today.
We helped the member order groceries for delivery and showed them how to do it again. We helped maintain transportation authorization and walked them through using the ride application. We connected them with a provider who better understood their diagnosis and worked through problems with broken or missing medical equipment.
We connected the member with an independent primary care partner. For a period of time, that provider saw the member weekly to understand their changing needs and assess whether physical therapy, occupational therapy or nursing support might help. Journey of Care also worked with the case manager to request personal care services and begin getting the apartment back into a safe, manageable condition.
Along the way, our team made space for grief, frustration and the loss of independence. We did not take control away from the member. We helped them regain it, one resolved problem and one achievable next step at a time.
03WHAT CHANGED
Not a perfect life. A more manageable one.
The result was food in the home, safer equipment, reliable access to care, a more manageable living space and a familiar team that kept checking back.
