SEND A REFERRAL

Use the secure pathway available to your team.

We welcome referrals from case managers, care coordinators, health plans, healthcare providers and community partners.

Portal

Anthem and other case management teams may have an established referral portal or approved internal process. Please use that pathway when it is available.

Secure email

Send an encrypted referral to contact@journeyofcare.com. Include the member's Medicaid ID and any information that will help us understand their care needs.

Fax

Fax referral information to (720) 463 1005. This is a good option when your secure email or portal is not available.

Please protect member information.

Do not place a Medicaid ID, diagnosis or other private care information in the general contact form above or in an ordinary unencrypted email. If you are unsure which method to use, contact us first and we will help.
