Journey of Care provides ongoing, personal support for people who need help navigating everyday life at home. A consistent team gets to know you, helps connect the pieces and stays available between visits.

WISCONSIN MEDICAID
Wisconsin Medicaid includes Remote Monitoring and Support within the Family Care and Family Care Partnership benefit packages for eligible members when the service supports long term care outcomes and is authorized by the member's MCO care team.
The service can provide access to live assistance from a remote caregiver and may include technology such as monitors, sensors or two way communication devices.
SUPPORTING THE CARE ALREADY THERE
Journey of Care works alongside personal care workers, home health agencies, therapists, healthcare providers, MCO care teams, families and other trusted supports.
It is to offer more consistent support and give the authorized care team better visibility into what is happening at home.

THE OPTIONAL TABLET
If you want a tablet and it is included in your authorized service, we send it directly to your home with cellular service and help you get comfortable using it. Home internet may not be needed.
The tablet uses large, easy buttons and can be comfortable even for someone who has never used a computer.
The tablet is not the service. It is one easy way to reach the people who are. You can also call, text or use another approved way to connect.

Some remote support models are built around a central call center. Journey of Care is built around small, consistent teams that know the member, notice changes and stay involved until the next step is clear.

We support the work you already do and share useful updates with the authorized care team.
AN INNOVATIVE WISCONSIN PARTNERSHIP
For eligible Anthem Wisconsin members, CSL Mainstay 360 connects in person care, home safety support and ongoing guidance in one coordinated program.
It is a flexible Wisconsin long term care benefit designed to help eligible members live safely and independently in a home and community setting. Mainstay 360 brings several kinds of authorized support together through one pathway.
01Provides in home personal care, completes the coordinated intake and serves as the front door and primary point of contact.
02Provides a familiar support team members can reach between visits, with practical guidance and follow through for changing needs.
03Provides a clinician guided home assessment and a personalized plan for safety, mobility, accessibility and fall prevention.
Talem activates the full Mainstay 360 program. The partners share information so the member is not left coordinating three unrelated services.
